Machine safety is often associated with one question: Is the machine properly guarded?
By itself, physical guarding is not a guarantee of safety. Workers who operate and maintain machinery still suffer roughly 18,000 severe injuries and more than 800 deaths every year.
That toll persists because traditional reviews stop at physical barriers, missing daily workarounds, overlooking improper lockout/tagout (LOTO) during jam clearing, and ignoring unmanaged line changes. A comprehensive machine safety assessment starts where those reviews stop. Here's what it examines.
Truth versus written procedure
One of the most valuable steps in evaluating machine safety is observing equipment during normal and non-routine work. Operators routinely interact with machinery to:
- Clear jams and pull scrap.
- Adjust tooling and reposition material.
- Clean components and perform routine setups.
Every one of those actions can bring hands and bodies close to points of operation, in-running nip points, rotating parts, belts and chains, crush points, stored energy, and unexpected movement.
A strong machine risk assessment asks whether the safeguard holds up against how people really work, not how a procedure says they should.
Recognize safeguard workarounds
Depending on the machine and the task, safeguards might include fixed or interlocked guards, light curtains, presence-sensing devices, two-hand controls, physical barriers, or other engineered controls.
It's just as important to read what the safeguards are telling you. Are the guards removed or damaged in any way? Do operators work around them because they slow production down? When a control is routinely bypassed, that's usually a sign it doesn't fit the task and the hazard it was meant to address is still there.
Integrate LOTO
Integrating LOTO requirements is a critical part of any machinery risk assessment, helping organizations identify hazardous energy exposures that may not be apparent during routine operations.
Guarding and hazardous-energy control tend to overlap. Servicing, maintenance, significant jam clearing, or any entry into a guarded area can call for LOTO, and this is one of the areas where employers get cited the most.
In the Occupational Safety and Health Administration’s (OSHA) financial year (FY) 2025 list of most frequently cited standards, LOTO landed at number four and machine guarding at number 10 across all industries. Narrow the view to machinery manufacturing (North American Industrial Classification System [NAICS] 333), and those same two standards sit at the very top: LOTO with 134 citations and machine guarding with 105, ahead of every other standard in the sector.
Manage change before modifying equipment
Machine safety is a continuous exercise. Even relatively small modifications to guarding, tooling, a conveyor, a sensor, an access point, an operating speed, or a production process can introduce a hazard that wasn't there before.
A formal management of change (MOC) process ensures that modifications are reviewed before they're implemented, new hazards are identified, risk is reassessed, controls are updated, and the affected procedures and training are revised to match.
Prioritize by risk score
Not every finding carries the same weight. A consistent risk assessment methodology evaluates hazards across machines based on:
- Severity of potential injury.
- Frequency of exposure.
- Likelihood of occurrence.
- Effectiveness of existing controls.
Done well, this delivers a site-wide picture of true machine risk alongside a prioritized punch list that directs budget and personnel to your highest-risk areas first.
Machine safety checklist
If you’re unsure where your safety program currently stands, evaluate your facility against these questions:
- Is there a site-wide machine risk assessment?
- Are the risks in a corrective-action or risk-reduction punch list?
- Are employees exposed during cleaning, setup, adjustment, or jam clearing?
- Do safeguards ever get removed, defeated, or bypassed?
- Do machine modifications run through MOC?
- Have LOTO requirements been reviewed machine by machine?
- Did the implemented fixes lower our risk?
